What is a Community Health Center?
A Community Health Center is a nonprofit organization, led by a community-based board of directors, that provides primary medical care and health-related services to individuals of all ages. Providers are typically family practitioners who can care for all members of the family. Community Health Centers generally offer additional services such as case management, health and nutrition education, chronic disease management and translation. They provide linguistically and culturally sensitive care and address the specific health needs of their communities.

Their goal is to promote good health through prevention, education, outreach and social services for the wellbeing of the patient and the entire community. Their purpose is to provide essential services to medically underserved individuals and families regardless of their ability to pay.
