Lamprey Health Care’s commitment to excellence, open communication, work-life balance, professional development, and overall quality of life is deep and genuine – and it begins at the top with our Board of Directors and Senior Management. Over the years, our Senior Leadership, Board of Directors and Staff have been nationally and regionally cited for their vision and leadership.
The Lamprey Health Care Board of Directors has continually invested in new facilities and health technologies. These investments facilitate the use of “best practices” that lead to better patient outcomes and continuity of care.
Our Newmarket and Raymond Centers were built to meet our unique needs, and both facilities have been renovated and expanded in recent years.
In 2000, Lamprey Health Care became one of the early adopters in the state to implement an Electronic Health Record (EHR) System. The EHR is central to the concept of a patient-centered medical home. Systematic use of information through the EHR has enabled our medical team to improve the quality and continuity of patient care, because the entire medical team has access to each patient’s medical record.
At Lamprey Health Care, every member of our team plays an integral part in our success.
As a patient-centered medical home, teamwork is more than a slogan at Lamprey Health Care; it’s truly the way we serve our patients. Each patient is assigned a team of providers that includes a physician, one or more nurse practitioners or physician assistant, and a registered nurse. These teams, in turn, involve our pediatrician, OB-GYN, nutritionists, diabetes educators, substance abuse counselors and care coordinators as needed to ensure continuity and comprehensiveness of care for every patient.
The Community Health Center “wrap-around” approach to health care, treating every patient as a whole person with a matrix of issues that affect their health, results in better patient outcomes. At Lamprey Health Care, we consistently outperform national and regional outcomes for prenatal patients and those with chronic conditions such as asthma and diabetes.
For over 35 years, Lamprey Health Care has provided high quality, cost-effective primary health care, with emphasis on prevention, education and social services. LHC has over 39 years of experience in assessing the needs of the communities we serve and adapting our services to meet these needs and reduce barriers to care.
In December of 2008, Lamprey Health Care was selected to pilot the Citizens Health Initiative Multi-Stakeholder Patient Centered Medical Home Program, a collaborative of practices that focuses on high quality, coordinated care, and standards of best practices that increase efficiencies and reduce healthcare costs for patients.
In May of 2009, we received recognition from the National Committee for Quality Assurance (NCQA) as a Patient-Centered Medical Home. The committee cited Lamprey Health Care for excellence in patient services, use of best practices, and integration of health information technologies.
Through this pilot program, Lamprey Health Care is taking teamwork, clinical quality, and coordinated care practices to a whole new level.
