Every Lamprey Health Care patient has a Care Coordinator who acts as an advocate to help navigate the health care system. Your Care Coordinator is available to answer questions, help with referrals, assist with follow-up appointments, and provide information & guidance to help you and your family get the additional supportive services you may need.

Whether it's asthma, diabetes or heart disease, our clinical team works hard to help you manage your condition for healthy results. Our doctors work as a team with our nurse practitioners, nurses and community health workers to ensure that you get excellent care management.
Our diabetes care program offers comprehensive diabetes education provided by an Registered Nurse Certified Diabetes Educator, according to the American Diabetes Association standards of care. Our goal is to provide patients with the information and tools they need to effectively manage their diabetes and lower the chance of developing serious complications.
Individualized diabetes education includes diabetes overview, nutrition, monitoring, medications, physical activity, complications, lifestyle changes, pregnancy and gestational diabetes. The Diabetes Care Team also includes physicians, nurse practitioners, registered dieticians and nurses.
Confidential substance abuse counseling is available at all three of our Centers. Our Substance Abuse Counselors provide comprehensive substance abuse services, including alcohol and other drug prevention education, assessments, referrals, individual and group counseling, and care management support.
At our Nashua Center, the Licensed Alcohol & Drug Counselor offers specialized services to pregnant women, parents, and women of childbearing age.
Good nutrition and physical fitness are key to preventing illness and injury. To help our communities choose and maintain a healthy lifestyles, we offer a wide range of programs, including educational cooking classes, senior exercise programs, yoga sessions and more.
Lamprey Health Care has Registered Dieticians on staff at each center to provide nutrition assessments and counseling to patients, including those who are pregnant or have diabetes.
Each Lamprey Health Care center offers interpretation services for our non-English speaking patients. In Nashua, the majority of our staff speaks more than one language which helps us to be more successful in meeting the needs of our patients.

Our Centers have contracted with Quest Diagnostics which provides on-site access to diagnostic lab services.
Most of your healthcare needs can be met by your primary care team in the comfort and convenience of our offices. For your specialized medical care, Lamprey Health Care physicians maintain strong referral relationships with their medical colleagues at Exeter Hospital and Southern New Hampshire Medical Center, as well as specialty hospitals in Boston, Manchester and Dartmouth.
To ensure that your health care costs will be covered, you should review and follow your insurance plan rules regarding referrals. These plans generally require that referrals be made by your primary care provider, so always contact us first before making an appointment to see a specialist. If you have no insurance, we’ll work with you to help you get the care you need.
