Many records are available through MY HEALTH, our secure online patient portal. Please ask a Patient Service Representative for a secure Personal Identification Number (PIN).
If you need a paper copy of your records we require a completed medical records release form.
Forms can be faxed or mailed to our office. Mailing address and fax numbers are listed on the forms.
Please allow 2-4 weeks for the records to be prepared for you.
For any questions regarding medical record requests please contact your center’s Medical Records Office by selecting option 7 through our phone system.
A personal representative is someone you authorize to be involved in your health care.
If you would like to authorize health center staff to speak with a spouse or another individual regarding your health care we will need to have this form completed. This includes conversations such as rescheduling appointments or questions on billing issues.
