Payment for Services

Controlling the cost of medical care is an important concern to all of us. At Lamprey Health Care, we strive to provide quality health care at a reasonable cost. Care is never refused because of a patient’s inability to pay.

The charge for your visit will depend on many factors, such as the complexity of diagnosis, laboratory work needed, and procedures performed. We encourage you to contact our business office if you have any financial or insurance questions.

Insurance
Please have your insurance identification card at each visit. Any deductible and co-pay amount is requested at the time of your visit. To ensure that your health care costs will be covered, please review and follow your plan guidelines. Some plans require that referrals be made by your primary care provider. Whenever you or a family member needs medical attention, you need to contact your primary care provider first. If your provider feels that a specialist outside of the center should see you, a referral will be made.

Lamprey Health Care accepts most health insurance plans but the following is a quick reference list (to determine if yours is accepted, please call us for more information):

Aetna US Healthcare
Anthem Blue Cross/Blue Shield
Cigna Healthcare
Health Care Value Management
Medicaid
Medicare
North American Administrators
One Health Plan
Private Health Care Systems (PHCS)
United Health Care

Financial Assistance for the Uninsured
If you do not have insurance coverage: We request payment for services at the time of your visit, however, care will not be refused if you are unable to pay. To assist you, we will offer a payment plan or a sliding fee discount for financially eligible patients. Our sliding fee scale program offers income eligible applicants a discount of 25% to 100% (less a minimum $15 payment or co-pay) off the cost of your medical care. Staff will provide you with financial assistance information at the time of your visit.

Our discount covers services provided by our health center only. Patients requiring labs, ultrasounds and other diagnostic or medical services at other facilities should request payment options from those facilities. We are unable to assist with those costs.

Most uninsured pregnant women are eligible for Medicaid. We accept Medicaid for our services and will gladly assist you in applying for Medicaid.

Prescription Assistance
If you are uninsured and need a prescription for an illness, we will try to assist with your medication needs whenever possible by giving you samples that have been given to us from pharmaceutical companies. Our supplies are usually limited, but we will do our best to accommodate your needs.

If you have ongoing medication needs for a chronic condition, you may be eligible for free prescriptions through our Pharmacy Assistance Program. If you are financially eligible, our staff will work to enroll you in programs where drug manufacturers provide you with free prescriptions. It takes four to six weeks to be enrolled and renewal applications are required every three months.
