Behavioral Health and the Primary Care Visit

Our Integrated Behavioral Health Program improves your access to behavioral services, resources, and counseling. Services are available for adults and children.

The team includes your Primary Care Provider, Care Coordinators, Behavioral Health Counselors, and Psychiatric Nurse Practitioners. The team works together using shared decision making and electronic health records. This allows us to provide the best care for you and your family.

When you meet with your primary care provider, issues may be identified where the Behavioral Health Care Team can provide support. Your Primary Care Provider may consult with the Behavioral Health Care team, or invite you to meet with them as part of your visit. They may also arrange for a future counseling visit. This flexible approach to care is based on your needs and shared decision making.

Counseling Visits

You can schedule an appointment directly with the Behavioral Health Care Team or through your Primary Care Provider. The Behavioral Health Care Team is here to help with:

  • Depression
  • Life issues
  • Anxiety
  • Substance abuse
  • Coping with chronic disease

For more complex Behavioral Health issues, our team will assist you with referrals to appropriate care facilities, so you can receive the care you need. Our goal is to improve your access to Behavioral Health Services in a safe and comfortable environment, through integration with your primary care medical home.

Substance Use Disorder Services

Lamprey Health Care will be launching a new program for Medication Assistance Treatment. This program will be for expectant mothers who are dealing with Substance Use Disorder. Check back for more information on this developing program.

Local Family Support Groups

Granite Pathways offers family support groups throughout the state to provide adult family members with a loved one affected by Substance Use Disorder. For full details on location and contact information, visit the Granite Pathways website. Meetings in our service area include:
