Care Coordination

Each of our health centers has a Care Coordinator who acts as an advocate to help patients navigate the health care system. Your Care Coordinator is available to answer questions, help with referrals, and assist with follow-up appointments. Our Care Coordinators can also provide information and guidance to help you access additional support and social services you may need.

Nashua Area Resources

The City of Nashua, in partnership with Partnership for Successful Living and American Medical Response, have started the Safe Station Program. People looking for assistance with Substance Use Disorder can go to any City of Nashua Fire Station to immediately receive services. Check out the video below for more information.
