Staff Spotlight: Carrie Chooljian

Director of Outreach and Care Coordination

LHC: Tell us a little about yourself. Where did you grow up? How did you get here?

Carrie:
I was born in Detroit, raised in Monroe, Michigan and after graduating from college with a Bachelors Degree in Social Work, I moved to New Hampshire to be near my then boyfriend Barry, who is now my husband of 37 years. Great decision! We raised two daughters In Hampstead, and both are now married, so I’ve gained two sons as well, along with a granddaughter Ruby.

LHC: What do you like most about working at Lamprey Health Care?

Carrie:
I love working with the amazing staff at Lamprey! Such awesome people!  I work in Raymond, Nashua and Newmarket, and it’s great to watch the staff come together to make sure our patients needs are being addressed. The additional services we provide, i.e. care coordination, behavioral health, diabetes education, and programs like New Hampshire Healthy Lives, make such a difference in a patients’ overall health.  I’m proud of the care we provide, and am especially partial to the Care Coordination department, which includes our Care Coordinators and Community Health Workers!    

LHC: Did you always want to work in healthcare? What would you do if you weren't doing this?

Carrie: 
I hadn’t planned on working in healthcare, but I am grateful that I’ve had the opportunity to work at Lamprey for over 17 years.  My early roles as a social worker were in child protection and family services, as well as human services for the towns of Plaistow and Hampstead.  I happened upon Lamprey’s ad for a Care Coordinator in Raymond in 2005, was offered the job, and it’s been a great fit since. I then accepted the role of Director of Outreach and Care Coordination over seven years ago, which includes supervising the Care Coordinators and Community Health Workers.  It’s provided me with the opportunity to work on new programs for our patients, including our van clinics, the Wisewoman and Healthy Lives programs, and support the staff on the care coordination team. 

LHC: What are some of your favorite things to do outside of work?

Carrie:
I love spending time with my family, and especially with my granddaughter Ruby, who will be 2 in May. I enjoy staying active, whether running, lifting weights, yoga, hiking, or snow skiing, as I know it’s made an inpact in my overall heath.  Trying to stay “young” too!  Lastly,  I love to read, listen to music, and travel, and just recently visited my daughter and son-in-law in Prague!

LHC: What advice would you give to others to be healthy?

Carrie: Set small goals to do something active every day.  It can be something small, and build from there!  Also, life gets hectic, so find time for “you” and things that bring you joy!

LHC: Is there anything else you'd like to share?

Carrie: I’m a board member for the Raymond Coalition for Youth, an organization I’ve loved supporting, for over 16 years. Their focus is on prevention and supporting youth to make healthy choices! Check them out at www.RCFY.org.
