Nashua: (603) 883-1626
Newmarket: (603) 659-3106
Raymond: (603) 895-3351
You can also call (877) 802-6079 to connect with our after-hours triage service, where you will have access to our on-call physicians.
For Newmarket or Raymond patients who are pregnant, have recently delivered a baby, or had a gynecological procedure, call (603) 778-7311.
Nashua prenatal and gynecological patients should call the after-hours triage line listed above.
If you are unable to keep your scheduled appointment, please call us at least 24 hours in advance so we can reschedule your appointment. Calling to cancel appointments will also free up a time slot for other patients.
Two days before your scheduled appointment, you will receive a text reminder. If you have opted out of text reminders or the main phone number you gave us is not a mobile phone, you will receive a phone call reminder. These calls may be automated.
With both text reminders and automated reminder calls, you will have the option to confirm, cancel, or reschedule your appointment.
Opting In and Out of Text Reminders
To opt-out of text reminders, reply STOP. To rejoin the text reminder system, simply text LAMPREYHC to 622622. For help with text reminders, text HELP.
Disclaimer: Message and data rates may apply. Consent is not a condition of purchase. This means that by using text reminders, you are not required to agree to any specific services from the health center.
To request refills of your current medications, please call your health center and request a refill at least one week before the prescription runs out. New prescription requests must be made through an appointment with your provider.
Advance Directives include “living will” and “durable power of attorney” forms that allow you to document your wishes. Making decisions about medical care is not always easy and having everything in writing will help your family when it comes to your care.
Aat some point, you may become unable to discuss your feelings with your loved ones – long before critical decisions must be made.
If you want to know more about Advanced Directives, please ask your provider, a Care Coordinator, or contact one of the local resources below.
