Appointments and Cancellations

If you are unable to keep your scheduled appointment please call us at least 24 hours in advance so we can reschedule your appointment. This will also free up a time slot for other patients.

Two days before your schedule appointment you will receive an automated reminder call. You will have the option through this automated call to confirm, cancel, or reschedule your appointment.

We also offer an opt-in text reminder service. To opt into the service text LAMPREYHC to 622622. For help with the text reminder program text HELP. To opt out of the program text STOP.

Disclaimer: Message and data rates may apply. Consent is not condition of purchase. This means that by opting into the text reminder program you are not required to agree to any specific services from the health center.

Prescription Refills

To request refills of your current medications, please call your health center and request a refill at least one week before the prescription runs out. New prescription requests must be made through an appointment with your provider. You can also request refills through our patient portal MY HEALTH. Ask a Patient Service Representative for a Personal Identification Number to create an account on MY HEALTH.

Advance Directives

Advance Directives include “living will” and “durable power of attorney” forms. These forms document your wishes regarding your medical care. Having your wishes in writing will help your family when it comes to your care. For information on Advance Directives ask your primary care provider or one of our Care Coordinators.
