Primary Care Patients

Prenatal Patients

For Newmarket or Raymond patients who are pregnant, have recently delivered a baby, or had a gynecological procedure, call (603) 778-7311.

Nashua prenatal and gynecological patients should call the after-hours triage line listed above.

Appointments and Cancellations

If you are unable to keep your scheduled appointment, please call us at least 24 hours in advance so we can reschedule your appointment. Calling to cancel appointments will also free up a time slot for other patients.

Appointment Reminders

Two days before your scheduled appointment, you will receive a text reminder. If you have opted out of text reminders or the main phone number you gave us is not a mobile phone, you will receive a phone call reminder. These calls may be automated.

With both text reminders and automated reminder calls, you will have the option to confirm, cancel, or reschedule your appointment.

Opting In and Out of Text Reminders

To opt-out of text reminders, reply STOP. To rejoin the text reminder system, simply text LAMPREYHC to 622622. For help with text reminders, text HELP.

Prescription Refills

To request refills of your current medications, please call your health center and request a refill at least one week before the prescription runs out. New prescription requests must be made through an appointment with your provider.

You can also request refills through our patient portal MY HEALTH. Ask a Patient Service Representative for a Personal Identification Number to create an account on MY HEALTH.

Advance Directives

Advance Directives include “living will” and “durable power of attorney” forms. These forms document your wishes regarding your medical care. Having your wishes in writing will help your family when it comes to your care. For information on Advance Directives, ask your primary care provider or one of our Care Coordinators.
