Services at Greater Nashua Mental Health Center

 

Lamprey Health Care’s Nashua Center and Greater Nashua Mental Health Center have partnered to bring you InteGreat Health, a collaborative program designed to make primary care and behavioral health services more accessible.

Patients in Nashua have the option to receive primary care services through the InteGreat Health program located at Greater Nashua Mental Health.

 

Whether a patient chooses our primary care office or InteGreat Health, they can access:

  • After-hours nurse call service
  • 24 Hour crisis intervention
  • Open access appointment scheduling
  • On-site pharmacy and lab draw services
  • Care coordination between agency providers
  • Timely substance use disorder referrals
  • Care management supporting patient goals

ProHealth NH

ProHealth NH is a person-centered health home for young people with Serious Emotional Disturbance (SED) or Serious Mental Illness (SMI). Providing integrated primary care, behavioral health, and wellness programs co-located in New Hampshire community mental health centers, provided through partnerships between the Community Mental Health Centers and the Federally Qualified Health Centers in Manchester, Nashua, and Rochester.

Integrated Care & Integrated Delivery Networks

Integrated Delivery Networks in NH are part of a first in the country project funded by a waiver from the Centers for Medicare and Medicaid. The purpose of the IDNs are to conduct demonstration projects to “Build Capacity for Transformation.” The project will go through 2020 and is focused on building a stronger healthcare workforce, increasing training and support for the workforce, and expanding health information technology (HIT).

There are seven IDN regions in NH. Lamprey Health Care is part of Region 3, the Greater Nashua Region, and  Region 6, the Strafford County/Seacoast Region. The regions are working to implement evidence-supported projects around three areas:

  • Mental health and substance use disorder treatment-capacity building
  • Integration of physical and behavioral health care
  • Improving transitions of care across settings

Lamprey Health Care’s integrated care programs, InteGreat Health, and Healthy Together are the result of the work of the IDNs.
